Septal Perforation Repair

Repair options for an opening in the nasal septum, according to its size, location and tissue quality.

Septal perforation is an opening in the nasal septum. Repair is a specialised operation planned according to the size and location of the opening.

What is septal perforation?

The septum is the wall that separates the two sides of the nose. When a hole connecting the two nasal cavities forms in this wall, it is called septal perforation.

Its causes include previous nasal operations, nasal trauma, cauterisation performed to control recurrent bleeding, the use of intranasal drugs or substances, infections and some inflammatory diseases. An effort is made to identify the cause.

Symptoms

Small perforations may cause no symptoms. Symptoms include crusting in the nose, bleeding, a feeling of blockage, a whistling sound when breathing and dryness inside the nose. With large openings, collapse of the nasal dorsum may be seen.

Assessment and treatment options

At the examination, the size and position of the opening and the quality of the surrounding tissue are examined. If there is an underlying cause, it is investigated separately; tests and opinions from related specialists may be requested when necessary.

If complaints are mild, non-surgical methods such as moisturising and care may be tried. Prostheses known as obturators are also among the options. If complaints continue, surgical repair is considered.

Surgical repair

In the repair, flaps are prepared from the surrounding mucosal tissue and a tissue graft is placed between them over the opening. The technique used is chosen according to the size of the opening and the condition of the tissues.

Not every perforation can be closed; success depends on factors such as the size of the opening and tissue quality. Expectations and limitations are discussed openly before surgery.

When should one seek assessment?

  • Persistent crusting and bleeding
  • A whistling sound when breathing
  • Nasal blockage or dryness
  • An opening noticed after a previous nasal operation

How does the process work?

  1. History and endoscopic nasal examination
  2. Assessment of the cause and the characteristics of the opening
  3. Discussion of non-surgical and surgical options
  4. Repair surgery
  5. Monitoring recovery and closure with check-ups

Frequently asked questions

Can every septal perforation be closed by surgery?

No. Small perforations that cause no symptoms can be monitored; closure may be difficult for large ones or those with poor tissue quality. The decision is made according to the examination.

Does a septal perforation heal on its own?

An established perforation is not expected to close by itself. Care methods can reduce symptoms.

Does a perforation spoil the shape of the nose?

With large perforations, collapse of the nasal dorsum may be seen. Small openings generally do not affect the external appearance.
